COVID-19 Vaccination Status Disclosure Form

Chamber Music | OC may require any individual appearing in person at its premises or otherwise participating in any event affiliated with Chamber Music | OC to complete and submit this form prior to their arrival.

COVID-19 Screening and Release Form

Chamber Music | OC requires all individuals who are not currently vaccine-eligible appearing in person at its premises or otherwise participating in any event affiliated with Chamber Music | OC to complete and submit this form no earlier than 24 hours prior to their arrival. If you are scheduled to appear in person, you may receive a courtesy reminder in advance. A new form must be completed and submitted for every appearance. There are no exceptions to this requirement.

Chamber Music | OC continues to actively monitor the COVID-19 pandemic. The safety of our students, faculty, staff, audience, and neighbors is our highest priority. Our new safety protocols reflect government directives, expert guidance, and established best practices for minimizing risk of infection.

Temperature

Upon entry to the building, visitors may have their temperature measured using an infrared, touch-less thermometer. If there is any evidence of a fever, individuals will not be permitted to enter.

Face Coverings

Face masks will be worn for the entire duration that an individual is on the premises. Both masks and gloves will be provided to any entrant that does not have their own.

Physical Distancing

A 6’ x 12’ banner, 18” x 12” posters, and a 12” x 12” walk-on floor sign will be placed in and around the entire premises reminding guests to implement social distancing whenever possible.

Plexiglass Dividers

Moveable plexiglass dividers may be placed between each Pre-College participant creating an added layer of protection against respiratory droplets.

Hand Sanitization

Hand sanitizing stations, Lysol disinfecting wipes, and FDA registered alcohol hand sanitizers will be available throughout the premises.

Air Purification

Dual HEPA filter air purifiers with smart technology capable of removing up to 99.97 percent of pollutants will be utilized in common areas. *

* Statement from the Environmental Protection Agency: When used properly, air purifiers can help reduce airborne contaminants including viruses in a home or confined space. However, by itself, a portable air cleaner is not enough to protect people from COVID-19. When used along with other best practices recommended by the Centers for Disease Control and Prevention, operating an air cleaner can be part of a plan to protect yourself and your family.
